Is spray foam waterproof on its own?

Closed-cell foam resists water absorption, but the waterproofing function is provided by the protective coating applied over it. The two are specified together as one system.

What thickness is required?

Thickness is set by the U-value or thermal transmittance required by the project specification and local energy regulations, not by a fixed figure — it is calculated per roof or wall build-up.

Can it be applied over an existing roof?

Often yes, provided the existing surface is sound, dry and adequately prepared, and any wet or delaminated areas are cut out first. A moisture survey is carried out before overlaying.
